29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. On or about December 1, 2016, the employer failed to keep clear the walking and working surfaces of the pipeline fitting department. In this department, the floor was not kept in an orderly condition as evidenced by several items that were sitting on the floor such as, but not limited to: a metal ramp that was laid over miscellaneous pipes and cords, other exposed pipes, cords, a floor trench drain and other debris, exposing employees to a tripping hazard.

29 CFR 1910.37(a)(3): Exit routes were not free and unobstructed. Materials or equipment were placed, either permanently or temporarily, within the exit route. The exit access went through a room that could be locked, to reach an exit or exit discharge. On or about December 1, 2016, the employer failed to ensure that emergency exits #53 and #54, located in Area G/Wet Goods Department (along the far East wall and in the maintenance section), were kept free from obstruction. One exit was blocked by multiple items such as pallets of materials and other miscellaneous items and the other exit was impeded by standing water on the floor and debris hanging from the wall over onto the top part of the exit door.

29 CFR 1910.151(c): 1910.151(c): At this establishment, the eye wash station, provided for employees exposed to battery charging operations, were not provided suitable facilities (eye wash containing a copious flow of water for a 15 min time period) for quick drenching or flushing of the eyes and body within the work area for immediate emergency use. On or about December 1, 2016, the employer failed to ensure that a plumbed in emergency eye wash station was accessible and kept in a sanitary condition in the Pipeline Fitting Department. The eye was unit was inaccessible due to the debris (cords, pipes, misc. other materials) in and around the unit and the spray heads of the unit were exposed which allowed dirt and other contaminants to rest upon them. Contaminants such as, but not limited to: paint and solvents.
