29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) On or about December 1, 2016, at the above addressed site, employees performing carpentry work were exposed to potential struck by eye injuries when using a Dewalt Drywall Screwdriver while not wearing eye protection equipment.  b) On or about December 1, 2016, at the above addressed site, employees performing carpentry work were exposed to potential struck by eye injuries when using a Makita 7 ¼ inch circular saw while not wearing eye protection equipment.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about December 1, 2016, at the above addressed site, employees performing carpentry work on an overhang were exposed to a fall hazard of approximately 10 feet 5 inches while not being protected from falling to the lower level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about December 1, 2016, employees performing carpentry work on an overhang were exposed to a fall hazard prior to being provided with a training program that would enable them to recognize the hazards associated with falls.
