29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  On or about December 5, 2016, at the worksite, an employee was preforming work from a job-made scaffold without fall protection.  The employee was exposed to fall hazards of over 10 feet.
