29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Auto Shop- General maintenance- On or about December 6, 2016 or times prior thereto, employees are exposed to potential inadvertent release of energy from an air compressor, drill press and bench grinder. This could result in a amputation hazard and/or temporary permanent disability.  (a) air compressor (serial # 00010913)  (b) Clausing drill press (serial # 520827)  (c) bench grinder

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy.  (a) Vehicle Compound- On or about December 6, 2016 and times prior thereto,  it was identified in the storage building that several 110V electrical outlets were damaged, and others were missing their cover plate. This exposed employees to electrical shock hazards from incidental contact.   The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(b)(2)(i) which was contained in OSHA inspection number 1055218, citation number 2, item number 3 and was affirmed as a final order on November 15, 2015 with respect to a workplace located at Prairie City Ranger District.

29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:  (a)  Vehicle tire storage area:  On or about December 12, 2016 and times prior thereto, steps were removed for repair 8 months prior, from a loading dock where employees regularly access the upper level.  Employees are exposed to a tripping hazard from carrying heavy items with no safe access.
