29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about December 15, 2016 and at times prior thereto, the employer did not have a written hazard communication program for employees who were exposed to bleach, degreaser and LP gas. This condition exposed employees to chemical hazards.

29 CFR 1904.1(a)(2): The company had more than ten (10) employees or more during the last calendar year, but did not keep OSHA injury and illness records and the establishment was not classified as a partially exempt industry under � 1904.2.  On or about December 15, 2016 and at times prior thereto, at the worksite located at 4444 Hwy 5, Benton, Arkansas, the employer did not maintain OSHA 300 logs and was not classified as a partially exempt industry. This condition delayed the timely assessment of workplace injuries during an accident investigation.
