29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) 1809 Washington St: On or about January 12, 2016 and times prior employees used a 100ft 14 gauge and a 75ft 14 gauge extension cord to supply power to a 7.5 amp Chicago Electric Low Speed Drill. The extension cords were daisy chained together to reach a length of 175 feet exposing employees to electrical hazards due to over current.
