29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.     a) Amboy Bank 95 Matawan Road, Old Bridge, NJ site - North side of the building; 2nd floor:       Employees performing construction and/or maintenance operations on the second floor of the building were exposed to an inadequately guarded open elevator shaft. The fall distance was estimated to be more than 10 feet high as the opening was from the second floor of building.    Condition noted on or about 01/19/2017.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:  a) Amboy Bank 95 Matawan Road, Old Bridge, NJ site - North side of the building:     Employees performing construction and/or maintenance operations on the first floor of the building were exposed to an unguarded open elevator shaft. The downstairs elevator opening was completely open exposing employees to an approximate 4 foot drop into the elevator shaft.  Condition noted on or about 01/19/2017.
