29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On the north side of the construction site, employees engaged in roofing activities were not wearing hardhats while working below other employees exposing them to being struck-by from falling tools and materials.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On the north side of construction site, employees engaged in metal roofing activities on a pitch roof of 10:12 without fall protection were exposed to an unprotected fall hazard of approximately 10 feet to the lower level.
