29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the holes. a.) On or about January 6, 2017, at 5941 South Noland, Kansas City, Missouri 64136: Employees engaged in snow removal activities on a greenhouse roof were exposed to fall hazards of approximately 20 feet. Floor holes on the roof, on which employees were walking, were not protected to prevent falls to a lower level. The employer failed to provide fall protection exposing employees to fall hazards during snow removal activities. An employee fell through a hole of the green house roof and was hospitalized with severe head injuries.

On or about January 6, 2017, at 5941 South Noland, Kansas City, Missouri, the employer failed to report within 24 hours the hospitalization of one of its employee who had suffered a severe head trauma due to a fall while performing snow removing activities.
