29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    On or about January 20, 2017, the employer failed to obtain written permission from the manufacturer or similar for the permanently attached additions they made to the Taylor Dunn powered industrial vehicle (maintenance utility cart, M5).  This includes, but is not limited to:  a permanently affixed large oversized metal maintenance tool box filled with tools, attached to the vehicles bed which overhangs the bed greatly,  and a vice installed on the front of the vehicle.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    On or before January 24, 2017, the employer failed to provide suitable eye wash facilities at the battery charging station area located in the rear of the facility.  There was a plumbed in eye wash unit with a shower head installed, but it was found to be in an unsanitary condition (very dirty ) as well as a two bottle hung unit (32 ounces each) that had expired approximately 9 months prior to the date of this inspection.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were not maintained in a legible condition:     On or about January 20, 2017, the employer failed to ensure that the identification label on the Taylor Dunn powered industrial vehicle (maintenance utility cart, M5) was kept in a legible condition so as to identify the make, model, serial #, load capacities and other important information related to the vehicle.
