29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):    (a)Signet Builders, Inc. @ 25036 438th St., Salem, SD 57058:  On and prior to January 18th, 2017, the exposing employer did not ensure a safety program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person. Employees were installing metal roof panels while exposed to a fall hazard of approximately 21 feet. On January 18th, 2017, an employee was walking across 2x4 wood purlins when a purlin failed and the employee fell approximately 20 feet. The employee was attempting to install insulation and was not using fall protection. This condition exposed employees to fall hazards of approximately 20 feet.        Abatement Note: OSHA defines a "competent" person as one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them.

An employee was installing insulation and metal roof panels without the use of a fall protection system. On January 18th, 2017, an employee was walking across 2x4 wood purlins when a purlin failed and the employee fell approximately 20 feet. The employee was attempting to install insulation and was not using fall protection. This condition exposed employees to fall hazards of approximately 20 feet.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:      (a)Signet Builders, Inc. @ 25036 438th St., Salem, SD 57058: On January 20, 2016, the exposing employer did not ensure the anchorage was designed and installed to maintain a safety factor of at least two.  Employees utilized a cross arm strap or d-strap wrapped around the apex of the top chord of truss and 2x4 purlin to install roof line insulation. The end of the strap continued and terminated on to the purlin. The connection point for the self-retracting lifeline and support during a fall event would allow the load to be transmitted to the 2x4 purlin which would not maintain a safety factor of at least two. The condition exposed employees to a fall hazard of approximately 20 feet.
