29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) On and before February 2, 2017, for the 2007 Ford F-250 Delivery Truck, the employer did not ensure the Eagle Lift Gate Attachment was equipped with guarding to prevent injuries created by the pinch points of the gate closing mechanism. As a result, one employee received a partial amputation to the left index finger while operating the gate unit.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours:      (a) On January 9, 2017, a work related incident resulted in the amputation of an employee's finger. The employer did not report the incident to OSHA until January 25, 2017.
