29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:   a) Arden Dinner Theater - On or about February 1, 2017, the employer allowed their sub contracted employees to stand on a wooden pallet  which was not attached to the forks of a "Gradall/All Terrain Fork Lift" elevated approximately 20 feet above grade.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers.    a) Arden Dinner Theater- On or about February 1, 2017, the employer's sub-contracted employees did not cover multiple floor holes that were observed on a walking/working surface being renovated in the dinner theater. The multiple floor openings ranged in sizes between approximately 6"x4" and 4"x4".
