Employees were exposed to a tripping hazard in that totes, plastic trimming, boxes, and debris/scraps from trimming activities were permitted to accumulate.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following, guardrail systems, safety net systems, or personal fall arrest systems:  On or about January 19, 2017, while on the platforms of the rotomolds, setting up dies, filling dies, and removing parts from the dies, employees were exposed to a fall hazard of approximately seven (7) feet to the lower level in that, when the platform is not extended there is no protection from falling to the lower level.

On or about January 19, 2017 in the rear of the facility near the compressor, where welding was once done, a single acetylene compressed gas cylinder was left free standing and unsecured in the rear of the facility. Employees in the break room were exposed to the struck by hazard when the load carrying capacity of the roof is unknown.
