29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Hospital Cooperative Laundry, at 6225 E 38th Ave., Denver, CO 80207: On or before February 9, 2017, the employer did not ensure that procedures were developed, documented and utilized for the control of energy when employees were performing maintenance on the Air Chicago Small Piece Folder machines, in that the employer's instructions for test and repair of the solenoids exposed the employees to drawing-in hazards due to running belts.. This condition exposed the employees to crushing and amputation hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) Hospital Cooperative Laundry, at 6225 E 38th Ave, Denver, CO 80207: On or before February 9th, 2017 the employer did not conduct a periodic inspection at least annually of the Lockout/Tagout (LOTO) procedures used by maintenance personnel. This condition exposed employees to crushing and amputation hazards associated with equipment such as, but not limited to the Air Chicago Small Piece Folder machines.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met: (a) Hospital Cooperative Laundry, 6225 E 38th Ave, Denver, CO 80207: On or before February 9, 2017, the employer did not develop, implement, and maintain at the workplace specific written hazard communication program which describes how the specified in paragraphs (f), (g), and (h) of this section for labels of warning, safety data sheets, and employee information and training will be met. The employer has not updated its Hazard Communication written program for the Global Harmonization System (GHS). This exposed employees to chemical hazards.
