29 CFR 1915.157(a): The employer did not ensure that employees used appropriate hand protection or protective clothing when exposed to hazards such as skin absorption of harmful substances, severe cuts, lacerations, abrasions, punctures, chemical burns, thermal burns, harmful temperature extremes, and sharp objects:  (a)  Port of Duluth-Superior, Roger Bough, ballast tank and/or number one cargo hold; On or about February 6, 2017, employees performing work with a hand-held torch were not adequately protected from molten metal, sparks, fire, or flame in that jackets and coveralls worn by employees were not fire retardant.
