(c)   LKQ Heavy Truck - On or about February 9, 2017 employees were exposed to a struck-by hazard due to a Tayler T-180 Forklift #13 serial no. S-BB -32409 having a cracked windshield. Some of these forklifts are driven and inspected daily but were not repaired or removed from service.
