29 CFR 1910.22(a)(2): (2) The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition.  When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided.   (a)  North Hallway near the Cylinder Storage Area - On or about February 9, 2017 employees were exposed to a fall hazard due to standing water on the floor. This condition has existed since the employer moved into the building in 2012.

29 CFR 1910.29(f)(1)(ii)(A): (A)The height of stair rail systems installed before January 17, 2017 is not less than 30 inches (76 cm) from the leading edge of the stair tread to the top surface of the top rail; and  (a)   Steps behind door leading Propane Tank Storage Area - On or about February 9, 2017 an employee was exposed to a fall hazard due to a handrail being broken and lying on the ground.  This condition has existed since the employer moved into the building in 2012.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.  (a)   LKQ Heavy Truck - On or about February 9, 2017 employees were exposed to a struck-by hazard due to a Toyota Forklift #11 serial no. 72526 not having a serviceable right front tire.    (b)   LKQ Heavy Truck - On or about February 9, 2017 employees were exposed to a struck-by hazard due to a Toyota Forklift #12 serial no. 10277 not having a serviceable right front tire.    (c)   LKQ Heavy Truck - On or about February 9, 2017 employees were exposed to a struck-by hazard due to a Tayler T-180 Forklift #13 serial no. S-BB -32409 having a cracked windshield. Some of these forklifts are driven and inspected daily but were not repaired or removed from service.
