29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      a) In the Production Area:  Employees feeding polyester sheet through the VAT section were exposed to a "caught-in" hazard from the S-drive rollers as energy control (Lockout/Tagout) procedures were not utilized, on or about February 6, 2017.    In the alternative:    1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc:    a) In the Production Area:  Employees feeding polyester sheet through the VAT section were exposed to a "caught-in" hazard from the S-drive rollers as machine guarding methods were not provided to protect employees from the ingoing nip point, on or about February 6, 2017.                                                                  ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedures at least annually to ensure that the procedures and the requirements of this standard were being followed:        a) At the facility:  The employer did not conduct a periodic inspection of the energy control procedures for equipment such as, but not limited to,  the Vat, Middle, and Winder (Line #2), at least annually to ensure that the procedures and the requirements of the standard were followed by authorized employees, as observed on or about February 15, 2017.                                                               ABATEMENT CERTIFICATION REQUIRED FOR THIS VIOLATION.
