29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  a) Plant Floor/Processing Line, Band saw: On or about February 16, 2017 and at times prior, the employer exposed employees to amputation hazards in that employees were allowed to change the blade of the Butcher Boy band saw without utilizing the lockout/tagout energy control procedures to prevent accidental startup of the machine.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) Plant Floor/Processing Line, Band saw: On or about February 16, 2017 and at times prior, the employer exposed employees to amputation hazards, in that employees were allowed to replace blades on the Butcher Boy band saw without first being trained to ensure those employees had the knowledge and skills for the safe application, usage, and removal of energy controls.

29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources:  a) Plant Floor/Processing Line, Band saw: On or about February 16, 2017 and at times prior, the employer exposed employees to amputation hazards, in that the employer had not provided authorized employees locks for isolating and securing machine energy sources in accordance with the established procedures.
