29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: (a) On or about February 14, 2017, Affordable Restoration, Inc., as the controlling employer, did not ensure that A&V Roofing and Construction (subcontractor) protected their employees from fall hazards greater than 25 feet while engaged in roofing maintenance operations at a residential apartment complex. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.23(c)(11): Where portable ladders were used to gain access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface: (a) On or about February 14, 2017, Fort Collins Restoration dba Affordable Restoration, Inc., as the controlling employer, did not ensure that A&V Roofing and Construction (subcontractor) employees safely used a ladder in that the ladder was not extended three feet above the landing surface. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
