29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Warehouse Floor, Machine Area - on or about March 3, 2017, employees performed servicing on a Travelling Head Hydraulic Cutting Machine, THNC-2570, but the employer had not established a program consisting of an energy control procedure, employee training, and periodic inspections to ensure the machine was isolated from the energy source and rendered inoperative to prevent unexpected energizing and startup or release of energy that could occur and cause injury.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: a) Warehouse, Machine Area - on or about March 3, 2017, employees performed servicing on a Travelling Head Hydraulic Cutting Machine, THNC-2570, adjusting the travelling head by reaching above the plate and engaging a switch to adjust the height of the head from the work table; the employer failed to ensure the control of energy by use of unplugging the cord and plug.
