All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).   On or about February 21, 2017, in the Oil Movement Process Area at Blanchard Refining Company, LLC located in Texas City, Texas; the employer failed to ensure the crude charge pump J27 was properly isolated for energy control prior to performing equipment decoupling.
