29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  Nytrex Industries, Inc., Macon, GA - In the mixing and production area, employees were required to use a half mask and full face, tight fitting respirators for protection against dust and vapors.  A written respiratory protection program had not been established, on or about 02/08/16.    Nytrex Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c)(1), which was contained in OSHA inspection number 1108630, citation number 1, item number 5a and was affirmed as a final order on 09/02/16, with respect to a workplace located at 507 Hillcrest Industrial Boulevard, Macon GA  31204.

A medical evaluation was not conducted, on or about 02/08/17.  Nytrex Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(e)(1), which was contained in OSHA inspection number 1108630, citation number 1, item number 5b and was affirmed as a final order on 09/02/16, with respect to a workplace located at 507 Hillcrest Industrial Boulevard, Macon GA  31204

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   Nytrex Industries, Inc., Macon, GA - In the production area, employees handle 49-70% hydrofluoric acid, 75% phosphoric acid, 50% caustic soda, 93% sulfuric acid, 50% sodium hydroxide, isopropyl alcohol 45% potassium hydroxide, 31% muratic acid, hexane, etc.  The employer had not developed a written hazard communication program, on or about 02/08/17.   Nytrex Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(e)(1), which was contained in OSHA inspection number 1108630, citation number 1, item number 8a and was affirmed as a final order on 09/02/16, with respect to a workplace located at 507 Hillcrest Industrial Boulevard, Macon GA  31204.
