29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a) On March 9, 2017 at W4659 Co Rd G in Black Creek, employees were using 110V receptacle to power an extension cord without the faceplate installed on the outlet box.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On March 3, 2017 at W4659 Co Rd G in Black Creek, the employer did not report to OSHA an employee hospitalization following a fall from height incident. An employee was admitted to a hospital after falling through a floor hole 9 ft. to lower surface level.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a) On March 9, 2017 at W4659 Co Rd G in Black Creek, employees were using a heater with damaged strain relief on the flexible cord; the cord insulation was pulled away from the fitting. b) On March 9, 2017 at W4659 Co Rd G in Black Creek, employees were using an extension cord with damaged strain relief at the plug end which exposed the conductors.
