F/V Frontier Mariner, Coastal Dock, engine room; on or about 07 March 2017 and times prior thereto; employee was not fit-tested annually for wear of North full-face air purifying respirator. Last entry was dated 06 January 2016.  This condition exposed employees to inhalation and ingestion of a hazardous substance.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve.  F/V Frontier Mariner, Coastal Dock, engine room; on or about 07 March 2017 and times prior thereto; North full-face air purifying respirators were not stored in a manner to provide protection from physical damage, environmental elements and contamination. Respirators were lying in work areas and /or hung on a wall peg. This condition exposed employees to inhalation and ingestion of a hazardous substance.
