29 CFR 1918.61(f)(2): Special stevedoring gear provided by the employer that has a SWL of 5 short tons or less was not inspected and tested as a unit, and certified, before initial use. (a) Weather deck: On or about March 8, 2017 and times prior thereto, the employer had no certification for two special stevedoring lifting devices. This exposed employees to struck-by hazards from not knowing the lifting limits on the gear.

29 CFR 1918.66(a)(3): Cranes and derricks having load ratings that vary with boom length, radius (outreach) or other variables did not have a durable load rating chart visible to the operator, covering the complete range of the manufacturers' (or design) capacity ratings. (a) Weather deck stern port side: On or about March 8, 2017 and times prior thereto, there was no load chart on the crane. This exposed employees to struck-by hazards from potentially overloading the crane.

(a) Gordon Jensen cranes: On or about March 8, 2017 and times prior thereto, employees were not performing daily crane inspections. This exposed employees to potential struck-by hazards from defects with the crane that might be missed from not performing daily inspections.
