29 CFR 1915.56(b)(4): When welding cable became worn to the extent of exposing bare conductors, the portion exposed was not protected by means of rubber tape, friction tape, or other equivalent insulation.  (a)  Maintenance area:  On or about March 8, 2017 and times prior thereto, a welding cable had multiple cuts to the conductor.  This exposed employees to shock hazards from coming in contact with the conductor.

29 CFR 1915.89(c)(7)(iv):   The lockout and tags-plus systems log did not contain the location of machinery, equipment, or system being serviced.  (a)  Engine space:  On or about March 8, 2017 and times prior thereto, work was being done on the hydraulic factory hydraulic pump and was not logged in the locks tag plus book.  This exposed employees to struck-by hazards from not knowing what system was locked-out.
