29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 11 March 2017 at 1640, an employee engaged in lubrication and preventative maintenance procedures on the Bread Molder machine labeled B-131 was exposed to amputation and caught in hazards in that energy control procedures were not developed and implemented to control the rotating chain and sprocket when the guard was removed.

29 CFR 1910.263(c)(5): Machinery that must be lubricated while in motion was not provided with stationary lubrication fittings that are designed to prevent employees from reaching into any dangerous part of the machine while lubricating. On or about 11 March 2017 at 1640, an employee engaged in lubricating the Bread Molder #1 labeled B-131 was exposed to amputation and caught in hazards in that no lubrication fitting was provided to keep employees from reaching into that dangerous parts of the machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and Employees engaged in daily operations and process monitoring at the bread oven were exposed to amputation and caught-in hazards in that a portion of the guard covering the oven sweep arm was not in place.
