29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tag out devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tag out devices, and other energy control measures:       The established lockout procedure for the Beer Column Reboiler, utilized during cleaning operations, was insufficient in that it did not address the following:       A.      Specific procedural control steps and measures for isolating and securing the thermal energy associated with the 150 degree hot water in the unit;   B.      Specific procedural requirements for testing and verifying the thermal energy control measures to ensure their effectiveness.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):       On or about March 14, 2017, employees performing service and maintenance on the Beer Column Reboiler did not complete the following procedures in sequence as required:       A. Physically reduce, dissipate, isolate or render safe the 150 degree thermal energy in accordance with 29 CFR 1910.147(d)(5).     B. Verify that the 150 degree thermal energy in the Beer Column Reboiler water was isolated and otherwise rendered safe in accordance with 1910.147(d)(6).
