This condition exposed the employees to entrapment, cave-in and explosion hazards.

29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means:    Excavation Site: On or about March 8, 2017; The employer did not determine by a safe and acceptable means the exact location of the gas line. The gas line was struck by the backhoe bucket being used to dig out the excavation. This condition exposed the employees to entrapment, cave-in and or explosion hazards.

This condition exposed the employees to entrapment, cave-in and/or explosion hazards.

29 CFR 1926.651(g)(1)(i): Where oxygen deficiency (atmosphere containing less than 19.5 percent oxygen) or a hazardous atmosphere existed or could reasonably be expected to exist, such as in excavations in landfill areas or excavations in areas where hazardous substances are stored nearby, the atmospheres in the excavation were not tested before employees entered excavations greater than 4 feet (1.22m) in depth:    Excavation Site: on or about March 8, 2016; the employer allowed two employees to work in excavation without prior testing the atmosphere inside. This condition exposed the employees to respirator hazards in excavation greater that 4-feet.
