29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:      a) 51 Mitchell Road, Ipswich, MA 01938.     On or about March 3, 2017, an employee working in the facility at a pallet repair station was exposed to continuous noise levels at 199.7% of the permissible daily noise exposure (8-hour time-weighted average sound level of 80 dBA) during the 475 minute sampling period on March 3, 2017; exposure calculations included a zero increment for the 5 minutes not sampled.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1904.19(d)

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    a) 51 Mitchell Road, Ipswich, MA 01938, Northeast corner of facility.    On or about March 3rd, 2017, the employer did not ensure that working space was maintained around a pair of electrical panels located adjacent to the wood fired heater, as the panels were blocked with a pallet loaded with wood, slowing employee access to the panels in the event of an emergency.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1904.19(d).
