29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a)  At the south side of the building  two employees installing glass mat faced gypsum panels (D-Glass) were exposed to a 19.4 feet fall hazard  while on the aerial lift (Mark: JLG; Model: 400S, S/N: 0300178564) without fall protection, as observed on or about 3/21/2017.
