29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    For the employees mixing chemicals such as, but not limited to sodium hypochloride into the drilling mud on or about March 20, 2017 and times prior there to.  The employer did not assure the self-contained eyewash station was in good repair, able to contain an adequate amount of water to flush the eyes. This exposes employees to the hazard of eye damage from not having an adequate supply of water to flush caustic or corrosive material out of the eye(s).

29 CFR 1910.23(b)(8): The employer did not ensure ladders were used only for the purposes for which they were designed.    For the employees climbing the ladder in the cellar on or about March 20, 2017 and times prior there to, the employer provided a self-supporting ladder to be used as an unsupported ladder leaning against the Blow Out Preventer for employees to climb to access the underside of the rig floor.  This exposes employees to the hazard of falling from a ladder.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.    For the employee mixing drilling additives into the drilling mud in the mud shack on or about March 20, 2017,  The employer did not repair the weather resistant cover for the 110Volt outlet by the eyewash station.  This exposes employees to the hazard of electric shock or arc flash.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws;    For the employees working on the rig floor on the off driller side, near the stairs to the mud pits on or about March 20, 2017.  The outer sheathing was degraded at the compression fitting allowing strain on the terminal connection on light number 32.

The employer did not use natural and synthetic fiber-rope slings that have permanently affixed and legible identification markings stating the rated capacity for the type(s) of hitch(es) used and the angle upon which it is based, type of fiber material, and the number of legs if more than one.  For the employees hoisting joints of drill pipe to the rig floor on or about March 20, 2017 and times prior there to, the employer did not remove synthetic slings from service that did not have legible identification and markings showing the capacity of the sling in its different configurations.
