29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Worksite - On or about March 23, 2017, an employee was operating a hydraulic shear, where the point of operation guarding had been partially removed exposing employees to a crushing hazard.    Abatement Note:  One acceptable means of abatement is to follow ANSI Standard B11.14; Section 8.1.2 Rear of the shear:  the user shall establish and ensure adherence to safe work procedures regarding the operations required at the rear of the shear. The user shall provide adequate protective measures to control the hazards at the rear of the machine which include, but are not limited to: a) moving parts; b) sliding or falling material which has been sheared;  c) rear entry to the point of operation; d) accumulation of material on the floor, conveyor or other material handling equipment.
