Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   a) On or about March 20, 2017: on a 3:12 pitch roof employees removed old roof shingles and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure, the employees were exposed to an 11-foot fall hazard.   b) On or about March 21, 2017: on a 3:12 pitch roof three employees were observed removing nails were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure, the employees were exposed to an 11-foot fall hazard.
