29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Jobsite, 231 Olympic Drive, Pittsburgh, PA 15236: On or before March 26, 2017, employees working at the bottom of a trench that was approximately 10 feet in depth with vertical sidewalls were not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. The employees were exposed to an engulfment hazard while engaged in work at the bottom of the excavation.

29 CFR 1926.652(c)(3)(i): Designs of support systems, shield systems, or other protective systems were not selected from and were not in accordance with tabulated data, such as tables and charts: a) Jobsite, 231 Olympic Drive, Pittsburgh, PA 15236: Observed on or about March 29, 2017, support systems fabricated from Georgia Pacific brand 7/16 inch Cat OSB (oriented strand board) sheathing and Stud Select brand kiln dried white wood 2X4's were not selected from and in accordance with tabulated data approved by a registered professional engineer. Employees working at the bottom of the trench (approximately 10 feet deep with vertical sidewalls) were exposed to an engulfment hazard.
