29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place:  Employees engaged in forklift operation activities, and employees near forklift in operation, throughout the facility were exposed to struck-by hazards. The employees were exposed in that the control lever(s) on the propane Toyota orange forklift Model-7FGCU18, SN-73063 was missing 2 out of 4 directional control markings.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, and rotating parts:  Employees engaged in linen folding activities at the Chicago Spreader Feeder machine #3 located in the Finishing area were exposed to struck-by and amputation hazards in that machine guarding was not provided and utilized. The rotating black and white belt was not guarded, employees were not provided protection from rotating parts and prevented from reaching in to clear jams while the machine is still running.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):  Employees engaged in linen folding activities and maintenance activities were exposed to caught-between hazards in that the following vertical belt and pulley were not guarded.   a) Finishing Department - On the side of the Chicago Ironer/Spreader Feeder machine #1, approximately 1 to 3 feet above the ground, pulley was not fully guarded.  b) Sorting Department - On the back side of the Milnor 135 (11470) Washer machine #1, approximately 4 feet above the ground, pulley was not guarded.  c) Sorting Department - On the back side of the Milnor 135 (11471) Washer machine #2, approximately 4 feet above the ground, pulley was not guarded.  d) Sorting Department - On the back side of the Milnor 135 (11472) Washer machine #3, approximately 4 feet above the ground, pulley was not guarded.  e) Sorting Department - On the back side of the Milnor 135 (11473) Washer machine #4, approximately 4 feet above the ground, pulley was not guarded.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
