29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration; Such activities were not performed only by experienced and trained employees selected for such work by the competent person:    a- Building M, 145 Cliffwood Avenue, Aberdeen NJ - An employee was exposed to fall hazards, in excess of 20 feet to the ground surface below, while performing siding installation from a pump jack scaffold that was without bracing installed 4 feet above the brace to be passed and was missing end stops.  On the day of the worksite inspection, this scaffold was not inspected for visible defects, prior to its use, by a competent person.  Condition observed on or about 04/05/17.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.452(j)(2): When the pump jack had to pass bracing already instaled,k an additional brace was not install approximately 4 feet (1.2 m) above the brace to be passed, and was not left in place until the pump jack was moved and the original brace reinstated:    a- Building M, 145 Cliffwood Avenue, Aberdeen NJ - An employee was exposed to fall hazards, in excess of 20 feet to the ground surface below, while performing siding installation from a pump jack scaffold that was without bracing installed 4 feet above the brace to be passed.  Condition observed on or about 04/05/17.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a- Building M, 145 Cliffwood Avenue, Aberdeen NJ - An employee was exposed to fall hazards, in excess of 20 feet to the ground surface below, while performing siding installation from a pump jack scaffold that was without bracing installed 4 feet above the brace to be passed and was missing end stops on both ends of the walking/working surface.  On the day of the worksite inspection, the exposed employee and the company owner had not received scaffold hazard recognition and minimization training by a qualified person.  Condition noted on or about 04/05/17.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.    a- Building M, 145 Cliffwood Avenue, Aberdeen NJ - An employee was exposed to fall hazards, in excess of 20 feet to the ground surface below, while performing siding installation from a pump jack scaffold that was missing end stops.  Condition observed on or about 04/05/17.     Best Option Construction Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(I), which was contained in OSHA inspection number 1112065, citation number 1, item number 5 and was affirmed as a final order on 04/07/16, with respect to a workplace located at 657 Chamberlain Avenue, Perth Amboy NJ.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
