OSH ACT of 1970 Section (5)(a)(1): OSH ACT of 1970 SECTION (5)(a)(1): The employer did not furnish employment and  a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:    On or about March 28th, 2017 employees operating and working in the area of the KOMATSU W250 Front Loader, Serial Number A65540, equipped with a forklift attachment, were exposed to struck-by hazards in that the equipment was not used in a safe operating condition.  The windshield was broken, the side window glass was broken and replaced with cardboard, and the side mirrors were broken and not replaced.  Among other methods, some feasible and acceptable abatement methods to correct this hazard may include, but are not limited to the following:  Operate the machine in accordance with the Manufacturer's Operator's instructions, which states in the safety chapter, page 2-15: "Check the mirrors on the machine before starting operations everyday.  Clean off any dirt and adjust the view to ensure good visibility", and "If visibility cannot be sufficiently assured, position a flagman if necessary".
