29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Jobsite, on or about April 6, 2017, employees who were not wearing head protection were working below and near work where a piece of angle iron was handled, exposing employees to struck-by hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: a) Jobsite, on or about April 6, 2017, an employee was allowed to use cross bracing and scaffold up-rights to ascend and descend the scaffold system, exposing the employee to fall hazards up to about 9 feet.
