29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Short Run Manufacturing - On or about April 8, 2017, the employer did not ensure that the point of operation was guarded on the Cincinnati Press Brake #D42, serial #52287, foot pedal activated, which resulted in a crushing injury to the right arm of one of the two press brake operators working at this machine when making bends in metal stock to form the shape of a box.    Trend Technologies, LLC was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA Inspection Number 1001534, Citation 1, Item 1, and was affirmed as a final order on April 29, 2015, with respect to a workplace located at 737 Fargo Avenue, Elk Grove Village, IL, 60007.

29 CFR 1904.31(b)(2): The employer did not record injuries and illnesses for employees obtained from a temporary help service, employee leasing service, or personnel supply service, and were supervised by the employer on a day-to-day basis:    Short Run Manufacturing - On or about April 8, 2017, the employer who supervised a temporary employee on a daily basis did not record an injury that was suffered by the temporary employee on their OSHA 300 entry log for the calendar year 2017.    a) On or about April 8, 2017, a recordable injury occurred when a temporary employee crushed his right arm when it became caught in Cincinnati Press Brake #D42, serial #52287.
