On or about April 13, 2017 - at the above addressed jobsite - employees working on the Third Floor near the trash chute were exposed to falls, due to a floor cover not being secured to prevent displacement, thereby exposing   employees to fall hazards of approximately ten (10) feet.

On or about April 13, 2017 - at the above addressed jobsite- employees working on the Third Floor near the trash  chute were exposed to falls, due to a floor cover not being properly labeled or marked with the words hole /cover, thereby exposing employees to fall hazards of approximately ten (10) feet.
