29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. a) Fort Eustis Building 1605: On or about 04/14/2017, the ground fault circuit interrupter three way pigtail used to supply temporary power to a battery charger and reciprocating saw was not used properly in that the ground fault circuit interrupter was not plugged directly into the permanent power of the building when used with a yellow 100 ft. extension cord. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage, sharp corners and projections, or pinch points. a) Fort Eustis Building 1605: On or about 04/14/2017, the yellow100 ft. extension cord used to provide temporary power to a reciprocating saw and battery charger, had been routed between a metal door and metal frame thus exposing the cord to a pinch point. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
