29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about May 1, 2017, the employer failed to ensure that effective machine guarding was present on the front (operator) side of the Sutton #3000 press. Barrier guarding was in place however employees were still able to reach hazardous areas of the equipment.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    On or about May 1, 2017, the employer failed to ensure that periodic inspections of the energy control procedures for the Sutton #3000 press had been performed. The procedures were developed in late 2015, and an audit of the procedures or personnel had not been performed since that time.    American Aluminum Extrusions was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1910.147 (d) which was contained in OSHA inspection number 1082232, citation number 1, item number 1b and was affirmed as a final order on 10/29/2015, with respect to a workplace located at 4416 Louisville Rd NE, Canton, OH.
