On or about May 3, 2017, at the above addressed jobsite, an employee conducting siding installation operations with a pneumatic gun was not provided with safety glasses, thereby exposing the employee to struck by hazards.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:     a.  On or about May 3, 2017 at the above addressed jobsite, employees were exposed to falls while performing siding installation from a ladder jack scaffold, nineteen (19) feet four (4) inches above the ground, due to the working plank extending over eighteen (18) inches from the supporting ladder.

On or about May 3, 2017, at the above addressed jobsite, employees conducting siding installation operations from a ladder jack scaffold approximately nineteen (19) feet four (4) inches above the ground were not provided with an access ladder, thereby exposing the employees to fall hazards.    Frame Pro Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1) which was contained in OSHA inspection number 442753, citation number 2, item number 1 and was affirmed as a final order on June 28, 2012 with respect to a workplace located at 2524 W. Medill Ave. Chicago IL.
