29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. At 1842 8th Street, St. Louis, MO, one employee was measuring the length of a gutter pan while working from a tubular weld standard frame scaffold. The working platform was 4.5 tiers high or approximately 22 feet above the ground below. A standard guardrail system or the equivalent was not installed along both ends or the rear. This violation was observed on 05/08/17.

29 CFR 1926.451(e)(1): When scaffold platforms were more than two feet (0.6 m) above or below a point of access, safe access was not provided: At 1842 8th Street, St. Louis, MO, one employee was measuring the length of a gutter pan while working from a tubular weld standard frame scaffold. The working platform was 4.5 tiers high or approximately 22 feet above the ground below. This employee climbed up a portable extension ladder to reach a portion of the frame, climbed over it, then jumped from one platform to another. This violation was observed on 05/08/17.
