29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  At or about May 10, 2017 and times prior thereto, at the worksite located at or near 304 Country Club Parkway in Maumelle, Arkansas, ground employees were picking up trash and discarded shingles without head protection while employees on the roof threw off old shingles. This condition exposed employees to struck by falling object hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about May 10, 2017 and times prior thereto, at the worksite located at or near  304 Country Club Parkway in Maumelle, Arkansas, employees were removing shingles from a residential house 6 feet or more high without any fall protection. This condition exposed employees to fall from height hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At or about May 10, 2017 and times prior thereto at the worksite located at or near 304 Country Club Parkway in Maumelle, Arkansas, employees accessed the roof of a residential house with a portable ladder that did not extend 3 feet above the roof. This condition exposed employees to fall from height hazards.
