29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours.  On or about 12 May 2017, the employer failed to provide copies of the injury and illness records to an authorized representative.  A request was made on 12 June 2017 and as of 20 June 2017 documents have still not been provided.
