29 CFR 1910.22(a)(2): The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided: (a). Worksite located at 2600 Maguire Blvd., Columbia, Missouri- Employees working near Tunnel #1 were exposed to a slip and fall hazard in that the floor around the area was covered in water due to the drain being too small to accommodate the water.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a). Worksite located at 2300 Maguire Blvd., Columbia, Missouri- The ends of the linen conveyor line were not guarded to prevent contact with rotating parts.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: (a). Worksite located at 2600 Maguire Blvd., Columbia, Missouri- Employees were exposed to a shock or electrocution hazard in that the employer had an extension cord run from an outlet and wrapped around the metal staircase to power an electric fan. The cord was being used in lieu of permanent wiring.
