29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about 5/10/2017, North side of the building:  An employee was cleaning on the 14th floor, no railings or fall protection was in use.  Employees exposed to falls of 14 feet.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  a) On or about 5/10/2017, North side of the building:  A flight of stairs located in stairway B between the 13th and 14th floor was not provided with a stair rail system.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems which met the criteria contained in Subpart M of 29 CFR 1926:  a) On or about 5/10/2017, North side of the building:  A landing on stairway B between the 13th and 14th floors was not provided with a guardrail system on the exterior side and was missing a midrail on the interior side.  Employees were exposed to falls of 7 feet to the interior and over 100 feet to the exterior.
